Page 1 of the Forms packet explains what each staff person must complete based on their position. All Forms must be completely filled out by staff member, parents, medical doctor, and school (CIT Work Permit).

- Work Permit – Must be printed on Canary Yellow paper. You will receive a copy in your CIT packet to take to school for signature. This is link is only to be used as a back-up.
